Kindergarten

• Must be 5 years old on or before October 31 of the current year
• Photocopy of PSA Birth Certificate
• 3 copies of 1x1 ID picture

Grade School (Grade 1 to 6)

• New Students
• Must be a kindergarten completer and at least 6 years old by enrollment
• Kindergarten Report Card
• Photocopy of PSA Birth Certificate
• 3 copies of 1x1 ID picture

• Grade 2 to 6 Transferees
• SF 9 (Report Card)
• Photocopy of PSA Birth Certificate
• 3 copies of 1x1 ID picture
• Good Moral Certificate

Junior High School (Grades 7 to 10)

• New Students and Tranferees
• SF 9 (Report Card)
• Photocopy of PSA Birth Certificate
• 3 copies of 1x1 ID picture
• Good Moral Certificate

All Returning Students
